3.8 · 110 reviewsI absolutely love and appreciate this place. My dad had a biapass and needed a rehab for recovery. I had to fly back to VA after his hospital discharge. This place took great care of him. They made him feel like family and got him back on the road to recovery. They stayed in contact with me the entire time even though I was in Virginia. He was there for 3 months. I highly recommend this for anyone going through rehad.
We are very happy with the care mom is receiving. Everyone we have come in contact with has been friendly, professional and kind. Mom is happy and thriving, and that's the most important thing. Every time we go to see her, she says, "I love this place!". That's a ringing endorsement!
I had the pleasure of having both my maternal grandmother and paternal grandfather stay at Jacksonville Nursing and Rehab Center, and I was truly impressed with the level of care they received. The staff was attentive and willing to go above and beyond to ensure their comfort and well-being. It gave me great peace of mind knowing that any concern was addressed quickly and with genuine care. I’m grateful for the support and dedication shown to my family during their time there.

The facility is clean and the staff is cheerful and polite. Everyone is very helpful and friendly. We went to visit a resident here during the ice cream social and there was music and plenty of ice cream for everyone, including us!
We moved my Aunt here from Georgia 2 years ago and the care has been AMAZING! So much so that even after our family had to move out of state, my Aunt stayed because she knew her care wouldn’t be the same. Dr Covington Director of Jax Rehab, is on top of any issue that needs to be addressed! She even made sure to insure that the key to beauty shop is available so I can do my Aunt’s hair lovingly like I used to do at home. Culinary director is also FANTASTIC! We surprised my Aunt with a visit, and was so happy to see fresh fruit and a beautiful salad with her dinner. Healthy options and not all processed! The Culinary director (I think her name is Ms. Cruz) listened to our request and makes sure to provide low inflammatory diet for my Aunt even when nobody is there to oversee. Social services also went above and beyond when we had to replace her broken glasses! Kudos for their help! Karaoke is fun too They work hard to keep it clean! Very grateful.Thank you❤️

My mother was admitted for short term rehab. The staff was very accommodating and patient with her needs. She had no complaints with her care or therapy. She felt very welcomed and loved on. Thank you.
My daughter Trena Rivers and myself Bobbie Davis for all the wonderful services we experienced while she was a patient at the facility. Precious, Artimzca and (All) the others CNAs, I can't remember all of theirs names was great! Thank you all so much, you deserve Gold Stars each of you! Also a special thanks to Mrs. Pat, her nurse, she also took excellent care of my daughter, thank you Mrs. Pat, you are an amazing person! To the THERAPISTS, all of you are wonderful. You are a great team! I watched how you worked with not only my daughter but other patients also, you have a heart in working with each patients, and special thanks to the head therapist for keeping me up to date. Special thanks to Trena Speech Therapist Kendall, I saw the work she put in to getting Trena back to eating and talking, thank you so much! I would also like to thank Nicole who is no longer with the facility, she was so helpful in making feel we made the right choice in choosing the facility, I will always be grateful for her assistance.
