Skilled Nursing in Jacksonville, with spaces to feel at home and support for your next chapter.
Ask about this communityFirst Coast Health & Rehab in Jacksonville, Florida, provides skilled nursing, rehabilitation, and short- and long-term care. The center says it has served the Duval County community for more than 40 years and is located close to local hospitals. Its services include post-acute care for people moving from a hospital toward home, as well as ongoing nursing and restorative care for residents who can no longer live on their own. The available information describes these as different care needs served at the same center.
For people transitioning from hospital care, the center says its team works with the physician to coordinate services toward the individual’s goals for discharge home. Rehabilitation is provided through physical, occupational, and speech therapy. These therapy disciplines are part of the center’s post-acute and rehabilitation services; the available information does not detail treatment schedules or the specific activities included in each therapy. Short-term care is also listed, though the pages do not describe how long an individual stay may last.
Nursing services are provided by registered nurses, licensed practical nurses, and certified nursing assistants. For long-term care, the center lists general nursing and restorative services. The description of these services is distinct from its post-acute focus: long-term care is for people needing ongoing support, while post-acute services address a transition from hospital care. The center describes its long-term setting as home-like but does not provide details about room or apartment layouts.
An interdisciplinary team develops individualized care plans for patients and residents, with input from the person receiving care, family members, and staff. The center describes this as a collaborative process to coordinate services around individual care needs. Care planning is presented as relevant to both post-acute patients and long-term residents, rather than as a service limited to one type of stay.
Other listed supports include social services, activities, nutritional services, housekeeping, and laundry. These are provided alongside nursing and rehabilitation, covering both support services and practical aspects of daily life. The available information names these services but does not specify menus, activity schedules, or the particular assistance offered through social services. A third-party facility profile also lists memory care, without explaining its specific services or how care is organized for people with memory-related needs.
That third-party profile mentions outdoor areas and resident activities and programs, but does not describe the outdoor features or name specific programs. The center’s own information emphasizes its proximity to local hospitals and its work with physicians for discharge planning. Taken together, the described care ranges from rehabilitation and coordinated post-hospital support to continuing nursing and restorative services. The pages do not specify living arrangements beyond describing the long-term setting as home-like.

Majority of these 5 star reviews are from people that work there. Even the administrator of the building has given it a 5 star review 🤣
great place for love ones😍
Sent my loved one there for rehabilitation and he ended up really needing long term care. The transition was so smooth and he was able to stay put. Team was nice and always was welcoming when coming in the door.
Don’t go buy what u seen online. This building really looks good on inside. Therapy team is very awesome. My admissions process was very smooth. You can tell the staff really cares.
The building has turned around completely. Staff really cares and want to make a difference in the residents lives
I'm very pleased with the care my brother has been receiving. Although he still clings to a few cherished delusions, he seems to feel safe and enjoys the food. My brother has suffered schizophrenia for many years and he can be a real burden. It is obvious that your care team and care plan have been effective. Especially by keeping up with his medications.
If I could give 0 stars I would. My grandfather is in this facility currently. We have tried reaching out to the SW for questions especially because we live out of state. I work in home health so I understand the role of a SW and this woman needs a new job. She has no answering machine so cannot leave a message and she never answers her phone. We e-mailed since we couldn't leave a message and no return e-mails. We finally got an e-mail today that stated we needed to figure out what the plan was for my grandfather to discharge which we didn't even know it was close to him to discharge since we cannot get a hold of anyone for information. She then called to say she got the email but didn't answer because she had already given us the answer, which she did not as we haven't spoken to her and was also told by her it isn't her job to help us out to find a facility in our state or transportation from FL to our state. Which 100% that is the job of a SW. Not sure who I can report this facility to but I will be reporting for how poorly it is ran. Also SW at my job is reporting this SW for neglect. Oh also the director doesn't take calls you must e-mail her too which she never emails back. What a joke!
So great the people are amazing oops wrong place
