About this community

Winter Haven Health and Rehabilitation Center is a skilled nursing and rehabilitation center in Winter Haven, Florida. It serves Winter Haven and Polk County, and the center says it is located close to local hospitals. Its services include post-acute care for people moving from a hospital toward home, as well as short-term and long-term care. The center also reports serving the area for more than 60 years.

For post-acute care, the center lists physical, occupational, and speech therapy alongside skilled nursing. Its description frames this care as support for people transitioning from a hospital, with the team working with a physician on services related to each person’s goals and planned discharge home. These therapies are part of a broader rehabilitation offering; the available information does not specify therapy schedules or particular treatment programs.

For people needing ongoing care, the center lists general nursing and restorative services as part of its long-term care. Its service list also includes respite care and palliative care, alongside short-term care. Memories Care is another named service; a third-party facility profile describes a Memory Care Unit. These offerings cover several types of nursing and supportive care, though the available descriptions do not explain eligibility or how each service is arranged.

The center describes care planning as an interdisciplinary process involving residents or patients, families, staff, and physicians. The team develops an individualized care plan rather than describing one standard plan for everyone. Registered nurses, licensed practical nurses, and certified nursing assistants are identified among the care staff. Social services are also listed, alongside nursing and rehabilitation, as part of the center’s services.

For everyday support, the center lists activities, nutritional services, housekeeping, and laundry. A third-party facility profile also identifies recreational activities, a library, and a beauty salon. These details indicate that the center’s listed offerings extend beyond clinical care to some shared resources and practical services. The available information does not give activity schedules, describe dining arrangements, or explain the layout of residents’ rooms.

Taken together, the service descriptions distinguish care focused on recovery after hospitalization from longer-term nursing and restorative support, while also naming respite, palliative, and memory care. The center’s stated planning approach brings family members and staff into care discussions with the physician and the resident or patient. Specific care needs and daily routines are not detailed in the available descriptions, so the listed services provide an overview rather than a full account of how an individual plan is carried out.

Covit-19. For the month of July 2020. There closed the only glass door to visit! I don't see any harm at all.... Now, you can't visit!! Hard to get someone in the phone 45 minutes or the phone gets cut off.. I been waiting for two weeks for administrative to called me back with the statues?Very disappointing 😕

This place is beyond horrible. I once timed the call lights to see how long they took to come and it was 45 minutes from the time I pushed it till someone came in. The food is HORRIBLE. It looks like slop and tastes just as bad. The housekeeping is non-exsitant- they are there but they don't keep anything clean at all. The halls stink of urince and feces. The CNA's let people go around with food on their clothes all day and not change them. The COIVD has really made these matters worse since family can't come in and see their family members. This is the WORST place I have ever been and I will…

This is the WORST place I have ever been and I will NOT ever go back. I would not send my worst enemy to this place. Please do your loved ones a favor and look at other facilities as this one should be shut down for not meeting the bare and basic needs of the peopel in there.

The worst place I’ve seen. The food is awful and if you turn you light on for help you could wait for hours. At night the CNAs go to the dining room and stay there. Lights were on all up and down the halls with people needing help. I honestly don’t know how they keep their license.

The staff at Winter Haven Health & Rehabilitation Center is very nice, but the place is very old, it smells, and it would not be my first choice for anything. Dad's room is old, the ceiling needs to be painted, the sink drain is blocked up, and the tiles are very old, but they try to keep it clean because housekeeping comes every day. They have a small activities room and a nice rehab center.

Winter Haven Health & Rehab was nice and very clean. They were accommodating and did what they needed to do.
