Assisted Living and Memory Care in Macclenny, with spaces to feel at home and support for your next chapter.
Ask about this communityW Frank Wells Nursing Home is a skilled nursing facility in Macclenny, Florida, in Baker County. Its care options include short-term rehabilitation and long-term custodial care, which differ in duration: rehabilitation is described as recovery support, while custodial care is ongoing nursing-home care. The provider says its nursing care serves people with varied needs, though it does not list specific long-term diagnoses or explain the clinical services attached to custodial stays. The facility is licensed in Florida as a skilled nursing facility and accepts private payment, Medicare, and Medicaid. Available information does not explain how coverage is determined or which costs each payment source covers.
Short-term rehabilitation is intended for people recovering after surgery, hospitalization, stroke, injury, or another medical problem or procedure. The provider says plans are customized to residents’ needs, with the goal of supporting a return to daily routines rather than promising a particular outcome. A facility profile lists physical, occupational, and speech therapy; the provider’s own page specifically describes access to physical therapy. W Frank Wells is directly connected to Ed Fraser Memorial Hospital, providing on-site access to physical therapy, laboratory, and radiology services. The available information does not specify therapy schedules or particular tests.
Other nursing services include IV therapy and wound care. A third-party facility profile also lists dementia and memory care, support with daily living activities, and palliative care, but does not explain what memory support involves or which everyday tasks staff may assist with. Hospice care is offered through a partnership with Community Hospice of Northeast Florida. The provider’s page presents it as a service for patients and families, without further detail about eligibility or arrangements.
Room information describes a range of studio layouts, without stating dimensions, furnishings, or room-specific features. A facility profile reports capacity of 69 beds; this is a capacity figure, not current occupancy or availability. Resident activities are also reported, but no specific programs or schedules are named. Public details about daily life therefore include studio-style rooms, activities, and the listed nursing and support services, rather than a defined daily timetable.
The provider’s website identifies a visitation policy as a separate resource, but the available page text does not give visiting hours or entry rules. Public nursing-home records identify the facility as a nonprofit corporation. Together, these details describe a skilled nursing setting offering short-term recovery, ongoing custodial care, and hospice, with hospital-connected access to selected clinical services. The available sources do not detail room features, activity programming, therapy frequency, or individual coverage arrangements, so those aspects of daily life cannot be specified here.
