“This facility is always clean, it has always been well maintained. My uncle has been in this facility for almost 5 years. I have had good communication, it could certainly be better at times. But the nursing staff starting at the top has been excellent and my uncle is NOT the easiest to care for!!”

I have had good communication, it could certainly be better at times. But the nursing staff starting at the top has been excellent and my uncle is NOT the easiest to care for!!
I was highly impressed by the facility, as well as the friendliness and professionalism of the staff. They truly go above and beyond to provide superior therapeutic treatment in a nurturing environment. My only disappointment—which seems common among similar facilities right now—is that they are quite short-staffed on the weekends, even for rehabilitation services. Aside from that weekend gap, my experience overall was positive and I still highly recommend them for their care.
My mother in law stayed there for a hospice respite. We sent her with 4 medications and she only came back with 3 of the 4. They mistakenly kept one which is a controlled medication. They never called ama let us know. Once I realized it was missing I called them and they acknowledge they had it. I asked if they could get it to me because this error was on them and I live in St. Augustine and they are in Fleming Island and gas is $4.59 per gallon. No one can tell me how they plan on getting this back to us. I keep getting the run around and they want the supervisor that works 11 pm to 8 am to…

It’s a pleasure to provide the Pruitt team at Fleming Island my highest endorsement. I spent 30 years as a Navy Nurse CAPT and am currently the Vice President for Transformation for Sharp HealthCare in San Diego. My father Carl Summers, a 90 year old Navy Veteran, was transferred to Pruitt in Fleming Island in April 2026 after a hip fracture with multiple complications. The Pruitt leadership, clinical care and teamwork has been the best I have ever seen in my 42 years in healthcare. The nurses, LVNs and CNAs, PT, OT, specialist Nurse Practitioners are compassionate and personable as they carefully tailor personal care for my father. The facility is designed around a beautifully landscaped courtyard, bridging nature with the clinical environment. Pruitt feels like home and is extremely clean and welcoming. I would like to thank the Pruitt team for embracing our family and friends as they deliver top decile care to my father.
My experience during my stay has been awesome,working with the therapists was a very rewarding, building my body back to a functioning individual. It was a rewarding experience building my body back up it made me feel worthwhile again I thank you all for making this process rewarding for me. I hope I continue to do what I learned here by taking pride in my body again. THANK YOU AGAIN FOR ALL THE PROFESSIONALISM OF THE THERAPISTS AND THE ADMINISTRATION HELP I RECEIVED!!!
The staff here has been AMAZING!!! Especially the administrator!! She was so caring and attentive during my difficult time.
Where should I start! The first time my mom had to go there everything was good! The second and third Time we’re not so good. The second time they said they couldn’t keep her because she was still sick from pneumonia she was sent from Baptist hospital so they sent her back to the hospital then after she recovered from that they sent her back to Pruitt which they never told us when they were sending her back back-and-forth over there! So this last time she was there they lost all of her clothes. I tried to get reimbursed from them for a shirt I had just bought her. She wore one time and they tell me because nothing was labeled or logged in they can’t reimburse me. And they don’t know what happened to her clothes. My point being if they would have emptied the closet with all of her belongings, she would’ve had her clothes with her I even had to go back for her glasses. so now I’m out $35 for a Shirt that she wore one time and that was to the Pruitt place and it took them over a month to call me back and for the lady to tell me that there’s nothing they could do without them documenting what she had in there and why they did not document what she had in there is beyond me so if you decide to use this facility, make sure you document everything you take there. Otherwise it will end up missing. Very upsetting I waited for a month to get that kind of an answer. Nobody would call me back even though Emily at the front office that answers the phone was very helpful and trying to get things done. Connie was the one that called me back and told me that they couldn’t do anything. I was passed around for a whole month for absolutely nothing very disappointing.
