Thanks to Naslean the Business Office Manager who worked consistently to get the right Medicaid insurance for him. She worked with us in securing the bed for him. She is very professional, very articulate, knowledgeable, helpful, determined, and customer service oriented. She was able to get him in long term care. Thank you Naslean for all your help! Earlier this year my brother was admitted to Fernandina Rehabilitation and Nursing Center in the short term care until a bed was made available for him in long term care. Thanks to Naslean the Business Office Manager who worked consistently to get the right Medicaid insurance for him. She worked with us in securing the bed for him. She is very professional, very articulate, knowledgeable, helpful, determined, and customer service oriented. She was able to get him in long term care. Thank you Naslean for all your help! The social worker and Steven the Administrator insisted that there were no beds available but there was a place in Nassau that they would see if they had a bed for him but neither gave me any information about the place nor the location. I spoke to Steven a few times about the situation in long term care he never got back to me on anything it was when I saw him in the hall on my visits to see my brother and confronted him , he would give me an answer or say I will look into it. My brother fell twice the first time he was taken to hospital emergency with a big bruise on his head and a cut under his eye constantly bleeding which required stitches. He did not know what happened. I was given at least three different stories by Steven and station 2 staff. The second time he fell I was given several different stories where he fell. He was taken to exray told no fractures he fell on his hips. Jeremy the nurse told me he was the one who picked him up. Then Ty/ station manager said Jeremy was not there that day. Then the DON said Jeremy was there but not at that station. I find that there are several very unfriendly, and lack empathy staff members and a few that seems to have the patience and understanding and compassion on the care of their patients. I have requested to speak to the doctor about my brother’s care and have not talk to him yet. I did speak to the NP. When my brother was in short term he had a large bed in long term a regular size bed. I ask Steven the administrator about the bed several time. I am still waiting on the bed. I have been in dining hall and notice the patients not enjoying their food. My brother complains that it upset his stomach. We take food to our brother every week. His clothes are always missing although his name is ion each piece. I pray for improvement and safety in the care of all the patients. I will continue to confront those in charge of the neglect and mistreatment of the patients and also inform them of the kind and caring staffs.

Great staff - very professional and knowledgable but also friendly and very attentive. Have had family member here and now a friend - both received great care!

I give the staff a 5 star but the conditions of the facility, the beds, bedside tables and furniture a 1star. The odor will take your breath away until you get adapted to the surroundings. For a facility in Fernandina Beach it is less desirable than any facility I have visited in our surrounding area. I wish for my family to be in a more respectful facility.
My brother was a patient in the long term patient area for 9 months prior to his passing. He found the staff to be friendly and compassionate. I want to especially thank three employees. Naslene in the Business Office for her professional help with helping us to understand what his financial obligations would be. She explained the workings of social security benefits and how the "system" works. She was very kind and respectful to my brother. Tammy in the Activities department was such a huge help with these things too. Jeremy was my brother's nurse. What can I say?? He was absolutely wonderful to him and became a dear and close friend to him. The staff is very kind overall. The facility especially in patient rooms should be cleaned more often and food is not very good.
I am very pleased with several nurses in rehab. Kathy, Amanda, and many others are compassionate and competent. Cole at the front desk is amazing. I had an awful experience with Sereana in social sevices. I was told,point blank, she did not have time to provide me a list of long term hospice care facilities for my loved one, after she promised to do that 4 days earlier. She said it was my problem and she did not have a bed for him , so I would need to figure it out because Medicare was going to stop paying. She was rude and said, “Use your phone, it’s a computer.” Nasaline was also rude and provided one option. Today, I was treated with respect by Latoya, in social services. She provided me with the information I needed to move forward and a huge weight was lifted.
Visited my Dad today this is an excellent Rehab! Everyone is so helpful and so nice!
