Assisted Living and Memory Care in Port Charlotte, with spaces to feel at home and support for your next chapter.
Ask about this communityDouglas T Jacobson State Veterans' Nursing Home in Port Charlotte, Florida, is a state-government nursing home serving veterans. The Florida Department of Veterans’ Affairs reported that it opened on April 1, 2004, and a CMS-based listing records 120 certified beds. The facility is in Charlotte County. Its profile describes nursing home care rather than independent or assisted living. Room layouts and a regular dining program are not reliably detailed, so no specific housing or meal arrangements can be described. The bed figure refers to certified capacity, not current occupancy.
Skilled nursing care is listed, along with rehabilitation and rehabilitative therapies. Physical therapy is specifically named among the care and programming, giving a clearer indication of the restorative services associated with the home. The available description does not set out therapy schedules, the circumstances in which rehabilitation is provided, or how an individual program is arranged. These offerings are service categories, not a guarantee that every resident receives every type of therapy.
Memory care, palliative care and pain management are also included in the list of services. Medication management and assistance with daily living activities are named as well, extending the described care beyond nursing and therapy to include support with medications and routine tasks. Together, the categories point to a range of clinical, cognitive and personal support, although the material does not explain eligibility, staffing or how these services are coordinated for residents.
The daily-life details provided are recreational activities and social engagement, alongside physical therapy and rehabilitation. This indicates that the home’s described programming includes both therapeutic and social elements. No particular classes, events, outings or recurring schedule are identified, so the description does not suggest a specific activity calendar. The available information also does not describe common rooms or resident room arrangements, keeping the focus on the care and programming that are named.
A map listing notes customer parking, wheelchair accessibility, restrooms, air conditioning, outdoor seating and Wi-Fi at the site. These practical features offer concrete points about access and amenities, separate from the nursing services themselves. The listing does not specify the design of accessible routes, the extent of outdoor areas, or whether Wi-Fi is available in resident rooms, so no further details about those features can be inferred.
Area listings name HCA Florida Fawcett Hospital and several retail destinations, including Publix, Sam’s Club, Lowe’s and BJ’s Wholesale Club. Chick-fil-A, Wawa and Regal Cinemas Town Center 16 also appear among the listed nearby places. These names provide broad context for Port Charlotte, but the listing gives no distances or travel times. They are local reference points, not onsite services or activities offered by the nursing home.

Director Joscelyn is unprofessional and unethical. Do not go here.
Stay away from this place until they change their director (Jocelyn). The director doesn't care about the patients or staff. All decisions are made based on money, regardless of care for the patients.
My mother was a hundred years old, and was ABUSED from day ONE!! NOT TOILETED, no professional care whatsoever! They literally sent her to the emergency room within three day's which she DIED from NEGLECT AND ABUSE! DO NOT SEND ANYONE THERE EVER.
Very unfriendly receptionist.
Needs some administrative support. All year long they waste a lot of money on agency staffing. 3 float ,monitor, 1 on 1 , 8 cna on the board such a waste on tax money.
Clean,plenty of workers. Friendly
