Community Care Center

My 88 year old mother stayed at this facility for two months for physical therapy rehabilitation. While the building itself is a bit outdated, overall the staff took good care of her. I visited daily to check on her, and her bed and clothing were changed every day. She was never left sitting in a soiled diaper for long periods, and she came home without any bedsores, which was very reassuring. The facility was cleaned in the morning and at night; however, I do wish they cleaned more frequently throughout the day, as there were times when odors were noticeable. I want to especially recognize some outstanding staff members. Aretha at the front desk was amazing. Kim, the RN, was exceptional. Kim truly cares about her patients and makes an effort to engage with family members. Melinda, the CNA, provided excellent care for my mother; my mom would light up whenever she saw her. Melissa, the social worker, was also very kind and communicated clearly and consistently. With some remodeling and updates to the facility, this could easily become a top-tier skilled nursing facility center.

I can’t say enough good things about Kim the administrator. Her leadership, compassion, and professionalism truly set the tone for the entire facility. Kim is always approachable, responsive, and genuinely cares about both residents and families. It’s clear she runs the facility with integrity and high standards, and it shows in the quality of care provided by the staff.

If I could give a -5 star review I would. DO NOT send your family here unless you can sit with them every day. My dad was there because of an amputation with rehab. He was there two weeks lost 23 lbs, has pneumonia, his catheter was nasty. Doesn’t matter what kind of wound it is they only change bandages on Tuesday. I called five times a day with no phone call back trying to get update on my dad. Never got a return call. I had to fly on from out of state to check on him to find him fully sedated with marks all over him yet I never got a phone call. Multiple physicians at Plant City hospital told me of multiple other cases of health problems from residents there and they had “multiple” cases they were dealing with. They said it should be shut down. Receptionist had a poor attitude and acted like everything was a inconvenience for her. Had stuff shipped from Amazon with dads name on it never got it he had to sit in other people clothes and at one point had the same clothes on for 3 days as if he was homeless! Most workers have a terrible attitude and they don’t care about the residents. I waited two hours for them to move my dad from chair to bed because his back hurt. They didn’t start caring about my dad till they saw I was around.

While visiting a friend at what I thought should have been lunch time the residents still had not been given any lunch at 1:30 when I left there. Just a cup of coffee most rehab/nursing homes feed lunch between 11:30 and 12:30. I did not smell any food cooking either so lunch would be cold most likely. Most centers like this you smell food cooking but I didn't smell anything so it was at least "clean" smelling but not a cleaning solution which was nice.
