The 3,300-square-foot therapy gym includes a therapy kitchen and specialized equipment. Listed services include physical and occupational therapy, speech and language pathology, and rehabilitation for post-cardiac surgery, geriatric, joint replacement, neurologic, vestibular, orthopedic and sports medicine needs. A medical director guides skilled nursing care, with physician assistants, therapists and clinicians involved. The team collaborates with acute-care providers during transitions between hospital and home. Meals are served in the dining room or by room service; housekeeping, linens and towels are provided. The area also has a whirlpool spa, salon and barber, lounges and a screened lanai.
Cypress is the long-term care area for people needing skilled nursing care around the clock. Its furnished private suites, also 352 to 405 square feet, include a private bathroom, small kitchenette and wall-mounted smart TV. Personal laundry, linens and daily housekeeping are provided, and residents receive three chef-prepared meals in the dining room. Licensed nurses and certified nursing assistants are on site 24 hours a day; physicians, nurse practitioners and other providers visit, and physical, occupational and speech therapies are available. Living rooms and a great room host events, entertainment and religious services. Cypress also has an accessible whirlpool tub and screened lanai. Activities can include baking, volunteering, resident council participation, educational brain games, music and crafts, with programming addressing social, cognitive, physical, emotional, occupational and spiritual dimensions.

DON’T SEND A LOVED ONE HERE: My mother was admitted to this rehab facility after knee replacement surgery, and unfortunately our experience with pain management has been extremely concerning. Multiple times, ( going on 4 in 2 days) she was left waiting extended periods for pain medication while in severe post-operative pain. On one occasion, she was told a nurse would return in 10 minutes, but she waited over an hour before receiving medication. Watching a recent surgical patient sit in tears because of repeated delays is heartbreaking and unacceptable. I understand healthcare staff are busy…

I understand healthcare staff are busy and emergencies happen, but communication, follow-through, and basic compassion matter — especially in rehabilitation after major surgery. I sincerely hope management reviews staffing, response times, and patient care procedures so other families do not experience the same situation.

I was there post hip replacement. I can’t say enough good things about the PT/OT staff including Loni, Danielle, Debbie, TODD, Yvonne, Cheryl and all the rest. The food was good but lower salt would have been better. The room was kept neat and clean- yay housekeeping. Most of the CNA’s were great. The full time nurses did not engage with me but the part time nurses were great. The PA John - if you can’t say something nice don’t say anything. Overall great experience.
My father was in the memory care unit for almost 2 years. Jen and the staff truly loved and took care of him. At first, he resisted going into any facility. But quickly he felt cared for and a connection to the other residents and the people taking care of him. I would get constant updates on his health and how he was doing socially. The place always was clean and organized, and the meals were good. Unfortunately, Alzheimer’s did eventually take him. But I was at ease knowing that as a family we provided the best care possible, and that was done through Hope Preserve.
I had a knee replacement and went to the Preserve. My physical therapist, Cheryl, was absolutely wonderful, professional and kind and absolutely knew what to do to get me moving and help me when there was an issue. She took over got the troops together and it was settled. The food needs a lot of improvement.
Only 9 residents per CNA, excellent care, always attentive, very clean and neat. Luna, the CNA, washed my clothes as needed without me saying a word, always popping in to say hello or if I needed anything. She was excellent! I was in the rehab for 20 days with a total reverse shoulder replacement. PT and OT , 2 times a day and they really worked me. Danielle, Todd, Rex and Elizabeth worked with me directly in an exceptional way. Rex helped me immensely with my strength and balance on my legs .The food was prepared by a professional chef and everything was made from scratch and a great presentation. I highly recommend The Preserve facility. They are an assisted living, rehab and memory care facility.
This is my second stay. I fell the 4the and broke my hip and pelvis.Ive been getting the attention I need. Love the food.
