About this community

Lilac at BayView is an assisted living facility in St. Augustine, Florida, in St. Johns County. Its listing reports 69 beds and a Limited Nursing Services specialty license. Care information names personal-care assistance, help with activities of daily living, and supervised medication assistance. These details identify the listed care category, capacity, and main areas of support; they do not describe an identical service package for every resident.

Personal care, assistance with daily activities, and medication supervision are identified as separate parts of the care offering. The profile does not specify particular tasks, schedules, or how support is arranged for an individual resident, so it provides a general outline rather than a personal care plan. The facility’s description presents this support as intended to help residents maintain or improve health and live as independently as possible; these are stated aims, not promised outcomes. Its Limited Nursing Services specialty license is listed separately from the assisted living category, without naming specific nursing tasks or explaining when they are available. The facility is listed as accepting Medicaid, but the profile does not explain eligibility, coverage, or payment arrangements.

Private and semi-private rooms are both listed, offering two room arrangements to compare. The available information does not describe room size, furnishings, bathrooms, or whether residents may choose a specific room. English and Spanish are listed as languages spoken. The profile does not detail interpreter arrangements or language-specific programming, but the two languages are stated as practical information about communication at the facility.

Meals are prepared by a licensed dietician, with individual dietary needs considered. This describes both the professional role in meal preparation and the attention given to residents’ dietary requirements. The profile does not provide a menu, meal schedule, or examples of specialized diets, so it does not establish which particular dishes or meal plans may be available.

The monthly activities plan includes social and leisure activities and is listed as available to residents. Arts and crafts is named as one option within a broader activity program, giving a concrete example alongside the wider social and recreational offering. The profile does not set out a calendar of events or state how often particular activities take place, but it does identify both creative activities and broader programming.

Scheduled transportation is listed for medical appointments, shopping trips, and social or leisure outings, and the facility profile says this service is provided to all residents. The source describes transportation as part of the monthly activities plan, connecting practical errands and appointments with recreational trips. It does not specify departure times, destinations, or how rides are scheduled. Taken together, the profile outlines support with everyday tasks, meals, activities, and travel, while leaving resident-specific care arrangements and daily schedules undescribed.

My husband was at this facility last year for a couple of weeks. It’s horrible, the staff could care less about the patients! It’s filthy, they NEVER cleaned his room or bathroom so I did it myself. Food is terrible. Physical therapy is terrible as well. They WERE not even giving him his meds for Parkinson’s properly or on time! I called them out on this! I was doing his laundry, yet his Tommy Bahama shorts and shirts were disappearing! One day when I visited him he had on someone else’s clothing…I was livid! Then He fell one evening and they did not even notify me, the next day I was there…

PLEASE STEER CLEAR OF THIS PLACE! They deserve zero stars and should be shut down! They even have a 2 star review on Medicare.gov Look it up!

The care my mother received from this facility was horrific! My mother was admitted on March 15. During her stay, the level of care she received was unacceptable. The care my mother received from this facility was horrific! My mother was admitted on March 15. During her stay, the level of care she received was unacceptable. She was left in wet clothing and bedding for extended periods, and no therapy had been initiated. At 4:00 a.m., I received a message from her that simply read “HELP,” which caused immediate alarm. We went to her right away and found her severely disoriented. She ultimately had to be readmitted to the hospital. What is especially troubling is that several staff members indicated they would not place their own family members in your facility. My mother’s roommate also expressed a desire to be taken home. No patient or family should ever have to experience this. Families place a great deal of trust in facilities like yours, expecting that their loved ones will receive attentive, compassionate, and professional care. That trust was not only unmet but broken. After removing my mother, every healthcare professional we encountered—including doctors, nurses, and therapists—expressed concern and disbelief that she had been in your care. Additionally, the manager on call that morning demonstrated a dismissive and unprofessional attitude, showing no interest in addressing the situation or taking accountability. We did not know the extent of these issues at the time, but we do now. I urge you to take this complaint seriously, investigate what occurred, and implement necessary improvements to ensure that no other family endures a similar experience.

The food is terrible it's cold when it comes out of the kitchen

Such a scary, understaffed place. We arrived in the evening and, although the doors were supposed to be locked and only opened by personnel, the doors opened for us (such horrible security). We only saw one nurse working the floor but there were several patients calling out for help and roaming the floors in their wheelchairs. It was something out of a horror film. Mom called for help and she was told someone would be right back - but nobody ever came. There was a lot of confusion about the paperwork and the kind of care she needed. Fortunately, she was only in there for a few days but even getting her out of there was a challenge. With the exception of one or two staff members, everyone (especially the director of nursing) was rude and dismissive. I feel so bad for anyone who has to stay in there for any kind of extended stay.

Bayview staff took great care of our 98 yr old mother and we are fussy with our mom's care!! She is precious to us!! The staff was friendly and personable!! I would recommend Lilac at Bayview to everyone!!

Staff is unprofessional Facility could use a good cleaning No one seems To be in charge or care
