Assisted Living and Memory Care in Orange Park, with spaces to feel at home and support for your next chapter.
Ask about this communityLife Care Center at Wells Crossing is in Orange Park, Florida, in a residential area overlooking Lake Wells. The community’s own location information places it less than three miles from Orange Park Medical Center, the Country Club of Orange Park and Orange Park Mall. These landmarks give a sense of its position relative to medical, recreation and shopping destinations, although transportation arrangements are not described. A facility listing reports a capacity of 120 beds; this is a stated bed count, not current occupancy.
Care categories listed for the center include skilled nursing, short-term rehabilitation, long-term care and memory care. Skilled nursing is described as available around the clock. Inpatient and outpatient rehabilitation are both included in the service information, alongside the separately listed short- and long-term care categories. The descriptions name these options but do not explain admission criteria, how care categories are chosen or whether a person’s care changes over time.
Physical, occupational and speech therapy are the named therapy disciplines. Nurses and therapists are described as developing individualized care plans for each resident or patient, with that person’s needs and goals guiding the plans. This is the center’s stated approach to organizing its nursing and therapy services, rather than a single regimen set out for everyone. The sources do not specify diagnoses treated, session schedules or expected outcomes.
Several listed amenities relate to time spent indoors. A dining room and lounge are named, as are a library or reading room and television. The list also includes telephone service, while art classes provide a creative activity. These details identify a mix of communal spaces, reading and viewing options, and organized activity; they do not provide a timetable, describe the classes or say whether every activity takes place daily.
Other listed services are laundry and housekeeping, personal care, and access to a beauty salon or barber shop. These practical features sit alongside the shared spaces and art classes in the facility’s amenities list, covering household routines and grooming as well as leisure. The source does not set out how often laundry or housekeeping is provided, what personal care includes, or whether arrangements vary according to a person’s care needs.
The available descriptions distinguish clinical services from features of daily life: skilled nursing and rehabilitation are care services, while the dining room, lounge, reading area, television and art classes are listed as amenities. Inpatient and outpatient rehabilitation are both specified, but room types, dining menus, outdoor facilities and activity calendars are not described. The profile therefore identifies the options and features by name without detailing a daily schedule or particular living arrangement.

Staff very friendly and personable. Facility well maintained. Interested in long term care for a relative. Were visiting friends today who like their living arrangements at Life Care Center at Wells Crossing.
We just moved a loved one to this facility earlier this week for long term care. The staff there has been very helpful, professional and compassionate. That includes everyone from the front desk, business office, doctors, therapists, nurses, and CNAs. Each of them is working to make this transition a positive one and our family greatly appreciates this.
I have been to lifecare several times and I always received very good care. Medicare paid for my stay I was only in for 30 days. tricare also helped. My husband is their now and they are taking very good care of him.
Great. I didn’t buy anything.
Staff nice, rooms and rest rooms could use upgrades. Medical information and updates are good.
Front desk was very professional and the direction to rooms was very understandable. interior was very clean staff is very knowledgeable and professional. I was really impressed with the attention of detail for their residents, especially Mr Fitz.
Very sweet, compassionate people, although very understaffed. The resident rooms could use some attention to detail. Old furniture. Overpriced, but would like to see some of that revenue go towards staff salaries and training.
Nice friendly staff and very clean.
