To start, I want to be abundantly clear.... the Occupational and Physical Therapy staff are perfect here, the wound care staff (Karla & Terry) do magnificent work and the SOLE reason for the two star rating. Megan, the assistant Social worker is beautifully helpful & kind. The staff within these departments are competent and why I am better and healing as well I am and have been. Outside of those two departments, the rest would get ZERO STARS and a hope of being closed by the state of Florida. The food should be classified as a biohazard. I'm a renal failure patient and this is not food I…

I'm a renal failure patient and this is not food I should be given at any time for my health. This does more harm to my system and kidneys.

I would NOT recommend this place to even bring their greatest enemy! It was dirty and unsafe! Couldn't get help when needed, roaches, mold, phones in room didn't work, medicine wrong, lack of communication all around! Got my loved one out as soon as I could and will not go back.

I am writing this review to share my experience with this facility while my loved one was under their care. Unfortunately, the conditions were deeply concerning and unacceptable. There were clear sanitation issues, including what appeared to be mold in toilets and on windowsills. The smell of urine was persistent throughout the hallways, which made the environment feel neglected and unsanitary. I also observed patients emptying urinals into trash cans, which raises serious concerns about both hygiene and supervision. Beyond the physical conditions, the level of care was equally troubling. There were patients left slumped over in their beds without attention, and many staff members appeared disengaged. It was not uncommon to see nurses and care staff on their personal phones, including FaceTime and calls, rather than actively monitoring or assisting patients. What stood out most was the overall atmosphere, staff was unhappy and overwhelmed, which unfortunately translated into the level of care being provided. Facilities entrusted with vulnerable individuals have a responsibility to provide a safe, clean, and attentive environment. Based on what I witnessed, this location is falling short of that responsibility and should be held accountable for improving both its conditions and patient care.
