About this community

Heritage Park Rehabilitation and Healthcare was a skilled nursing facility in Fort Myers, Florida. A 2021 news report described it as a 120-bed nursing home. Federal regulators ordered a permanent shutdown after the facility failed to remedy longstanding patient-care and safety deficiencies; Medicare reimbursement for its care services was scheduled to end on August 14, 2021. The report said state records showed inspection violations dating to 2012 and that the facility had been on Florida’s watch list under a conditional license since February 2019. Reported issues included residents’ rights, staff competency, food storage and sanitation, and physical-plant standards. The facility had also not submitted required details for an emergency power plan.

A community profile lists skilled nursing and describes nursing services for 12 to 16 hours a day, a 24-hour call system, and 24-hour supervision. Other listed support includes medication management and help with activities of daily living, such as bathing, dressing, and transfers. Non-ambulatory care is also included in the profile. These are descriptions from an online listing, not confirmation that the services remain available.

Listed room features include furnishings, private bathrooms, kitchenettes, cable television, telephones, air conditioning, and Wi-Fi. The profile also names housekeeping, laundry or dry-cleaning, move-in coordination, concierge service, and family support. An emergency alert system and transportation appear among the listed amenities. The available information does not specify room-sharing arrangements, so these features do not establish whether rooms were private or shared.

The dining information describes a dining room with meal preparation and service, restaurant-style dining, and all-day dining. A professional chef is listed, along with special-diet options that include allergy-sensitive and diabetes diets. The profile names activity and gathering spaces such as an arts room, activity room, game room, library, and business room. It also lists a fitness room and programs, a spa or wellness space, a movie theater, and outdoor areas with a garden and walking paths.

Program listings include scheduled daily activities, music, movie nights, outdoor programs, and resident-run and community-sponsored activities. A separate location listing placed the facility next to Lee Memorial Hospital and near Fort Myers’ River District. Other profile information identifies nearby medical services, a Walgreens, St. Cecilia’s Catholic Church, and several restaurants. These references describe the surrounding area, but do not establish current access to services or amenities at the facility.

The 2021 shutdown order is the clearest dated information about the facility’s operating status. Later community listings still display service and amenity details, but the available sources do not establish whether Heritage Park operated after the scheduled end of Medicare reimbursement. Accordingly, the profile-listed features should not be taken as confirmation that the facility currently operates or offers those services.

Sad sad sad place. Find another facility. Executive Director is gone. Director of Nursing is gone. Director of Physical Therapy is gone. All within a short time. Now they have failed the State Inspection.

This place is awful! My mom was there last summer and acquired scabies, which they repeatedly lied to me about. She is still in quarantine because of ongoing treatment to eradicate them at her current (good) facility!! They have a terrible roach problem, their staff is very slow to respond, and one time told me to go around the corner so I wouldn't hear them talk. They lied about the medicine they gave her- she ended up in the hospital because of it. DO NOT STAY THERE!!

The staff was nice, and everybody was nice at Heritage. It was nice and clean, and there was no smell or anything like that; it was alright.
