Grand Oaks of Jensen Beach

Grand Oaks on Jensen Beach is one of the best ALF I have seen. They have great rooms and wonderful staff.

My grandmother was a resident at Grand Oaks on the Memory Care side. I am writing this review to share my experience, one that was deeply disappointing, heartbreaking, and at times completely unacceptable. She entered the facility more aware than she was by the end of her life, and I visited her frequently throughout her time there. I paid close attention to the quality of care she received, and unfortunately, the Memory Care side of Grand Oaks falls significantly short in several critical areas. To start, there was a noticeable lack of basic hygiene and resident care. My grandmother experienced frequent UTIs which ultimately escalated to sepsis, a consequence that could have been prevented with routine hygiene practices, such as daily changes of underwear/diapers. I often found soiled bed sheets left unchanged, raising serious questions about how often residents are being checked on, especially since staff are supposed to assist them every morning and throughout the day. On top of that, I observed many times that staff were sitting on their phones rather than tending to the residents. Some would even be “taking care of a resident” but were clearly just sitting on their phones, offering no interaction, no comfort, and no presence. For residents in memory care, simple companionship and attentiveness is essential and easy. This lack of engagement was so disappointing. After family reached out expressing concerns to the director of Grand Oaks you would think there would be change and improvement, CONSISTENT change and improvement. I was left to not trust them one bit. The lack of responsibility, the neglect in basic care, and the disregard for cleanliness left me feeling that my grandmother was not safe or properly cared for. To families considering Grand Oaks for memory care: please think twice, maybe even three times. The emotional toll of seeing your loved one neglected, paired with the lack of trust in staff to carry out even basic caregiving responsibilities is simply not worth it. I strongly recommend installing a camera if you decide to place your loved one here, only to have some peace of mind and ensure they are receiving the care they deserve. This was my family’s experience, and I share it in hopes that others can make a more informed decision.

Grand Oaks Jensen Beach is an amazing Assisted Living, the staff always go above and beyond to make sure the residents are comfortable, and their needs are met. The activities are great. Everyone always has a smile and is always willing to help.

I can say a lot of positive things about Grand Oaks - Jensen Beach. I loved that the nursing staff seemed to know every single patient by name. And they explained to us that they would get to know them, know all their needs, all their likes, and all their dislikes. It's a smaller facility so they can keep track of everybody in that way. As for the sales staff, they were not pushy at all. Everything was black and white. There were no sales tactics there. It is what it is. They welcomed us, and if it wasn't right for us, they were perfectly fine with that, too. And I like that. I appreciated that. If you have someone who has dementia, or an ALS patient, go check this place out. I was pretty impressed with that. I think it was quite clean there. I did not sample the food, but I toured the dining room while people were seated there, and each person who needed assistance appeared to have someone assisting them. So that was nice to see as well, to make sure that they weren't just throwing the food down in front of them and leaving them. They were helping them, which I thought was nice.

I can say without any doubt this is the by far the best community. Yes a community, not just a rehabilitation center. The staff are the most compassionate people I have ever met. Your loved one will treated with dignity and respect. The place is amazing in every way imaginable.
